This document contains personal information about the client, in client-friendly language, that can be used to help the client to communicate with new people; the booklet can be shown to new people the client encounters. Personal information includes: the client’s family’s structure, the parent/s’ short- and long-term hopes/goals for the child, likes and dislikes, health considerations, communication strategies and capabilities.
