I would like to share the form of assessing the situation of the child and family, which we used in our projects.
The idea of the document is that after activities to identify children with disabilities (together with social workers, our specialists visited families using door-to-door method), information is collected on unidentified children who need social assistance and support. And based on this completed form, an Individual Child Protection Plan and Action Plan is developed, which will guide the social worker to help solve problems and family’s difficulties. And it is important that the information reflected in the assessment will be available only to professionals who help the family and the child.
